Person and Family Engagement

Who we are

 Superior Health Quality Alliance (Superior Health) is a regional Quality Innovation Network – Quality Improvement Organization (QIN-QIO) under the direction of the Centers for Medicare & Medicaid Services. Superior Health’s Person and Family Engagement (PFE) team partners with health care players, patients, residents, family advocates and community allies on strategies that promote person-centered care. To ensure these voices are incorporated into efforts to improve care in organizations across the care continuum, Superior Health established the Person & Family Advisory Council (PFAC).  

How we help

As a foundational framework, PFE threads through all Superior Health focus areas. Guided by the Superior Health PFAC, the QIN-QIO works with nursing homes, hospitals, physician offices, home health agencies, and communities to implement meaningful strategies that advance person-centered care across this continuum.  

Superior Health creates more equitable and person-centered systems by seeking counsel from persons and families from diverse backgrounds, ensuring that healthcare change and quality improvement reflect the needs of all populations.

 

Why engage?

Engagement, education and empowerment of beneficiaries, which includes:    

  • Making informed decisions. 
  • Finding shared preferences and values. 
  • Working together to create goals. 
  • Encouraging meaningful engagement, education and promotion of self-management skills. 
  • Promoting person and family engagement practices in care delivery design by encouraging the use of beneficiaries and health care advocates as advisors on quality improvement committees and projects and/or Advisory Councils.      

Involvement and partnership with beneficiaries as advisors, which includes:    

  • Involving the voice and counsel of diverse beneficiaries and their health care advocates in
    the design of the improvement, based on their experiences.    
  • Incorporating stories and lessons from beneficiaries and their health care advocates in quality improvement activities.  

Promotion of compassionate and culturally competent care, which includes:  

  • Ensuring that care is compassionate, respectful, inclusive and reflective of people’s needs from different backgrounds, including race and ethnicity, religion, sexual orientation, gender identity, age, financial situation, physical and mental ability, and schooling.
