End Stage Renal Disease Network

End Stage Renal Disease (ESRD) Networks support the federal government in assuring appropriate care for patients who receive treatment through dialysis facilities and kidney transplant centers certified by Medicare. As one of 18 regional ESRD Networks in the United States, Midwest Kidney Network (ESRD Network 11) serves:

  • 5 states: Michigan, Minnesota, North Dakota, South Dakota and Wisconsin
  • 512 renal dialysis providers
  • 21 renal transplant centers
  • 48,187 people with kidney failure (as of 1/17/2017)

ESRD Improvement Initiatives 2013 – 2020

CMS set the following priorities for health care quality improvement to be implemented by its ESRD program, from August 2014 through July 2019:

Better Care for the Individual through Beneficiary and Family-Centered Care

  • Patient and family engagement
  • Patient experience of care
  • Appropriate access to in-center dialysis care
  • Vascular access management
  • Reducing healthcare-acquired infections

Population Health Innovation Project

  • Increase vaccination rates
  • Improve care coordination
  • Improve transplant coordination
  • Promote home dialysis (when appropriate)
  • Improve quality of life
