Nursing Home Targeted Response Quality Improvement Initiative

Nursing homes in the Targeted Response Quality Improvement Initiative (QII) work hand-in-hand with Superior Health Quality Alliance’s (Superior Health) quality improvement advisors to break down barriers, implement improvements and track and sustain progress.

  • Receive one-on-one support on infection control and COVID-19 best practices.
  • Access to hundreds of educational resources and tools.
  • A free onsite infection prevention and control assessment from nationally renowned infection preventionist.
  • Automatically be enrolled as a participant in the Quality Innovation Network-QIO (QIN-QIO).

IPCWell Team

IPCWell Team

As an active participant in this initiative, we offer a free onsite visit by an infection preventionist from IPCWell. Dr. Buffy Lloyd- Krejci and her expert staff provide in person training, education and technical assistance to long- term care facilities. When the IPCWell team enters the building, they spend two to four hours touring and conversing with staff to learn how infection program works. This is intended to be very informal and a relaxed unique learning experience and questions are always encouraged. Focusing on policies and procedures is not the priority at the visit, time is spent to understand infection control and prevention behaviors in daily operation. The Focus of Our Work

Superior Health is powered by eight organizations with proven success driving achievement of Medicare quality improvement program goals. Superior Health will work to improve the quality of health and health care for health care consumers, patients, clinicians, health care organizations and communities by supporting these five areas:

  • Improve nursing home quality focusing on reducing unnecessary resident harm, reducing hospitalizations and increasing a facility’s Five Star Quality Rating.
  • Increase quality of care transitions through engagement of community coalitions to reduce unnecessary emergency department visits and increase medication safety.
  • Increase chronic disease prevention and self-care for those living with cardiovascular disease, diabetes and chronic kidney disease.
  • Increase patient safety including reducing adverse drug events and healthcare-related infections.
  • Improve behavioral health and opioid misuse through prevention and access to health services.
