Superior Health Quality Alliance (Superior Health) is a regional Quality Innovation Network – Quality Improvement Organization (QIN-QIO) under the direction of the Centers for Medicare & Medicaid Services.
Superior Health’s Chronic Disease Management team focuses on assisting our participants to effectively manage chronic health conditions. Our goal is to help communities achieve improved outcomes and better health for the people served through the use of best practices and interventions to encourage active patient participation in their care.
The Chronic Disease Management Team will assist you by focusing on interventions tailored to meet your organization’s needs. Those who participate in our educational sessions leave with resources and tools to put in place immediately in the communities they serve. Our interventions and resources include cardiovascular care – including blood pressure management and control – diabetes management, tobacco cessation, patient participation in cardiac rehabilitation and slowing the progression of chronic kidney disease.

This six-part program provides detailed instruction about early identification and management of chronic kidney disease.
Blood Pressure Competency Training
This training helps demonstrate that staff are trained and can effectively perform blood pressure measurements. It also helps ensure that blood pressure data readings are accurate – an essential part of assisting patients to achieve improved blood pressure management.
