Care Coordination

Who we are

Superior Health Quality Alliance (Superior Health) is a regional Quality Innovation Network – Quality Improvement Organization (QIN-QIO) under the direction of the Centers for Medicare & Medicaid Services (CMS). Superior Health’s Care Coordination team is committed to improving hospital admission and readmission rates by working with all levels of care. Care Coordination also focuses on emergency department over-utilization rates by addressing patients living with chronic conditions and patients accessing the emergency department for non-medical services.

How we help

Superior Health will work with health care and community organizations to identify root causes of unnecessary admissions, readmissions, and emergency department over-utilization to find data-driven, person-centered, cross-setting improvement plans. We will provide relevant best practices, educational resources and tools in formats best suited to your organization’s needs. Superior Health will use evidence-based strategies such as person-centered care planning, medication reconciliation across care settings and care transitions communication tools to address care coordination challenges.

One such initiative is the Superior Health Regional Home Health Care (HHA) Workgroup. This group of dedicated HHAs and related organizations comes together monthly to evaluate emergency department over-utilization root causes. By sharing challenges, best practices and data, this group is currently developing targeted interventions to address and support high-risk patients from using the emergency department as their primary source of health care.
