This 30-minute presentation will describe the development of the Nevada Sepsis Coalition Toolkit for skilled nursing facilities and provide an overview of the individual tools in the toolkit. The Sepsis Toolkit is designed to support skilled nursing facilities in early recognition and intervention for signs and symptoms of sepsis in residents.
Sepsis is one of the top diagnoses involved with readmissions throughout the Great Plains and Lake Superior Quality Innovation Networks. This second webinar in the series introduces a toolkit developed after the successful implementation of an early recognition and management of sepsis program in post-acute care settings.
