Sepsis is one of the top diagnoses involved with readmissions throughout the Great Plains and Lake Superior Quality Innovation Networks. This second webinar in the series introduces a toolkit developed after the successful implementation of an early recognition and management of sepsis program in post-acute care settings.
The toolkit contains evidence-based, best practice education on protocols that assist in timely and effective interventions which reduced 90-day readmissions and mortality in the community in which the program was implemented. The program and toolkit use an integrated team approach with all health professionals, including certified nurse assistants in nursing facilities.
Audience
Primarily skilled nursing facilities, but with content for all health care settings related to statistics, screening, and progression of sepsis.
Objectives
- Describe the value of using an integrated team approach, from the medical director to the nursing assistant, in the implementation of a sepsis program
