IPF Sharing Call Series #2 | Risk Stratification and Integrated Care Perspectives in IPF Work

Join Lake Superior QIN and HealthInsight for the second sharing call in our Inpatient Psychiatric Facilities (IPF) sharing call series. The second call will include a discussion on Intermountain Healthcare System’s successful implementation of an integrated behavioral health model to reduce emergency room and IPF readmissions. This call will also focus on the use of implementing a risk stratification tool designed specifically for IPFs. Following a brief presentation on this topic, time will be available for IPFs to share their successes and ask questions of this system and other IPFs regarding their efforts towards reducing readmissions.

Join Lake Superior QIN and HealthInsight for the first sharing call in our Inpatient Psychiatric Facility (IPF) sharing call series. The first sharing call will provide an overview of three models that can be used in the IPF setting to reduce readmissions: the Bridge model, Project RED (Re-engineered Discharge Toolkit) and RARE (Reducing Avoidable Readmissions Effectively). Following a brief presentation on this topic, time will be available for IPFs to share their successes and ask questions of other IPFs.

Webinar | Strategies for Reducing Readmissions in the Inpatient Psychiatric Facility Setting

Join Kay Jewell, MD, healthcare consultant and National Alliance on Mental Illness (NAMI) volunteer, and Sue Abderholden, MPH, Executive Director of the NAMI Minnesota, for an insightful discussion on leading causes of readmissions in the inpatient psychiatric facility (IPF) setting and strategies to reduce them. Dr. Jewell will address the leading causes for readmissions across the three states in the Lake Superior Quality Innovation Network (Lake Superior QIN) region and discuss available community resources that improve behavioral health outcomes. Sue will discuss strategies learned while implementing a statewide campaign to reduce IPF readmissions in collaboration with Stratis Health and the Minnesota Hospital Association.
