Join Lake Superior QIN and HealthInsight for the second sharing call in our Inpatient Psychiatric Facilities (IPF) sharing call series. The second call will include a discussion on Intermountain Healthcare System’s successful implementation of an integrated behavioral health model to reduce emergency room and IPF readmissions. This call will also focus on the use of implementing a risk stratification tool designed specifically for IPFs. Following a brief presentation on this topic, time will be available for IPFs to share their successes and ask questions of this system and other IPFs regarding their efforts towards reducing readmissions.
