Community Care and Services

Superior Health recognizes the importance of community care in improving health and health care for community members.

Our team of experts work with community organizations to form partnerships, develop resources and guide decisions that make a local impact.

Our team not only focuses on developing resources to improve health, but we also focus on building culturally and linguistically appropriate services (CLAS) standards and building a dedicated Person and Family Advisory Council (PFAC) to guide all the resources we create. Ensuring the work we do is person-centered and developed with racial, ethnic and linguistic minorities, as well as people with disabilities and sexual and gender minorities in mind is paramount to improving care at a global level.

Community organizations working with Superior Health are provided:

  • Opportunities to work with real-time data.
  • Assistance with the evaluation of data.
  • Opportunities to be a part of large-scale systems of change.
  • Opportunities to use data to help your group focus on shared areas of improvement and provide relevant national best practices, person-centered educational resources and tools developed with the help of a dedicated PFAC.
  • Access to subject matter experts on quality improvement processes, CLAS standards and regulatory reporting requirements.
  • Learning opportunities offered in a variety of ways that fit skill sets and are easily accessible.

Community Health Disparities Reduction Partnership

Superior Health launched the Community Health Disparities Reduction Partnership in May 2024 to invest in projects in Wayne County (Michigan), Ramsey County (Minnesota) and Milwaukee County (Wisconsin) aimed at reducing disparities in health care outcomes in these communities.

Free Interactive Combined Vaccine Program

The Interactive Combined Vaccine Program is a dynamic education program designed to educate Medicare beneficiaries and communities about recommended vaccinations including: COVID-19, RSV, pneumonia and influenza in a fun and engaging way. The program can be adopted at your organization to help reduce the incidence of infections in communities.
