Superior Health collaborates with clinics across our service area to focus on systemic initiatives that improve the quality and efficiency of care for people with Medicare.

We offer one-to-one technical assistance along with broad scope learning opportunities to provide comprehensive solutions to our participants. For clinics, we provide assistance for chronic disease management, including cardiovascular care, diabetes management and chronic kidney disease care. Our participants will find that focusing on measures related to chronic disease management concurrently strengthens their ongoing efforts to successfully participate in federal incentive payment programs, such as Quality Payment Program (QPP).
