Nursing Homes

Superior Health is driven by eight organizations with proven success in advancing Medicare quality improvement program goals.

Our Nursing Home Quality Improvement Collaborative offers:

  • Data-driven technical support to drive impactful change.
  • Custom education, tools and resources to enhance efficiency and expertise.
  • Expert guidance to help you meet and exceed regulatory standards.

Superior Health quality improvement advisors work hand-in-hand with nursing homes to break down barriers, implement improvements, and track and sustain progress. We offer specialized 1:1 technical assistance tailored to meet your facility’s specific needs, ensuring our support is personalized to drive meaningful change.

Through this collaboration, you’ll gain insights into elevating your nursing home’s quality and implementing seamless transitions of care. Plus, your organization can access assistance in key areas such as: vaccinations, emergency preparedness, behavioral health and substance use disorder, chronic disease management and prevention, patient safety, patient and family advisory councils, health care leadership and reducing preventable hospitalizations and emergency department use.

Onsite Visits

As an active participant in this initiative, we offer a free onsite visit by an infection preventionist from IPCWell. Dr. Buffy Lloyd- Krejci and her expert staff provide in person training, education and technical assistance to long- term care facilitiesWhen the IPCWell team enters the building, they spend two to four hours touring and conversing with staff to learn how infection program works. This is intended to be very informal and a relaxed unique learning experience and questions are always encouraged. Focusing on policies and procedures is not the priority at the visit, time is spent to understand infection control and prevention behaviors in daily operation. Learn more about the Onsite IPC Visit experience by watching this video:
