The webinar will provide attendees with an overview of the cholesterol and hypertension impact on patient and community outcomes. Community engagement and ways to improve your control rates will also be discussed.
This webinar will provide information on the 2017 Hypertension Guidelines released by the American Heart Association and the American College of Cardiologists. The audience will learn how these guidelines assist in controlling hypertension through accurate blood pressure measurement, acting rapidly to minimize clinical inertia and partnering with patients, families and communities to improve patient engagement in their care.
The American Heart Association and American Medical Association partnered to launch Target: BP in 2015 to improve blood pressure control and build a healthier nation. This national initiative aims to reduce the number of Americans who have heart attacks and strokes by urging medical practices, health service organizations, and patients to prioritize blood pressure control. Based on the most current AHA guidelines, Target: BP supports physicians and care teams by offering access to the latest research, tools, and resources to reach and sustain blood pressure goal rates of less than 140/90 mmHg within the patient populations they serve.
This learning session will focus on the Centers for Medicare & Medicaid Services (CMS) hospital incentive programs.
Join Lake Superior QIN and the American Medical Association's (AMA) Linda Murakami, RN, BSN, MSHA for a discussion of the AMA's three-phase evidence-based framework for improving hypertension control rates in your patient population. The Measure, Act, Partner (M.A.P.) framework helps organizations deploy effective strategies for blood pressure measurement, protocol use and patient engagement that lead to improved control rates for patients with hypertension.
