Health Information Technology

Health Information Technology (HIT) is transforming health care by enhancing communication across care settings, improving patient outcomes and empowering communities. By integrating advanced technologies, HIT ensures that patients receive equitable, timely and effective care, improving access and fostering a connected and informed health care ecosystem. 

Electronic health records (EHRs) and patient portals are vital to patient-centered care, offering access to medical records, care summaries and secure communication channels between patients and providers.

These tools ensure continuity of care and actively engage patients and caregivers in managing their health. 

Electronic Health Record (EHR)

EHRs provide real-time access to comprehensive patient information, enhancing care coordination, supporting informed decision-making and enabling patients to actively participate in their care. This ultimately leads to better health outcomes and increased patient satisfaction. The Certified Health IT Product List, displays all health information technology certified for use in the CMS Quality Payment Program.

Patient Portals

Patient Portals enable better patient access to their health information, improve interoperability and unleash innovation, while reducing burden on payers and providers.

Health Information Exchanges facilitate the secure exchange of health data across different systems, promoting interoperability and ensuring providers have access to comprehensive patient information, regardless of where care is delivered.

Community Health Information Exchange includes health-related social needs and local resources. Real-time data sharing supports coordinated care and reduces redundancy. State-specific options and technologies further enhance this process.  

Health Information Exchange (HIE) / Community Health Information Exchange (CHIE)

Examples of technology embedded in HIE include: Push and pull queries, Admission, Discharge, Transfer (ADT) Notifications, and Critical Results Notification (Lab, Radiology, Pathology). CHIEs include patient screening and assessments for health-related social needs and closedloop referral management systems for communitybased interventions. 

  • Michigan’s statewide Health Information Exchange MiHIN is a network consisting of thousands of Trusted Data Sharing Organizations offering comprehensive data exchange solutions.  
  • Minnesota’s e-Health Initiative Minnesota e-Health Initiative is a public-private collaborative to advance the adoption and use of electronic health records (EHRs) and other health information technology (HIT), including health information exchange (HIE). 
  • Wisconsin’s statewide Health Information Exchange WISHIN has multiple technology solutions embedded in a real-time vendor-agnostic interoperability platform. 

Electronic health information exchange (HIE) allows doctors, nurses, pharmacists, other health care providers and patients to appropriately access and securely share a patient’s vital medical information electronically—improving the speed, quality, safety and cost of patient care. 

Clinical Quality and Technology Solutions

Clinical Quality and Technology Solutions are designed to improve healthcare quality and efficiency. Together, these technology solutions improve patient and population health outcomes, decrease costs, and enhance patient satisfaction. 

Quality Reporting and Quality Measures track, analyze and enhance care delivery by systematically evaluating health care outcomes. Clinical Decision Support (CDS) systems guide health care providers with evidence-based recommendations, enhancing outcomes and streamlining workflows. Clinical registries track health trends, inform public health interventions and offer insights into population health. Remote monitoring and telehealth extend care beyond traditional settings, allowing for real-time patient monitoring and reducing the need for in-person visits. Remote Patient Monitoring, Telehealth and Mobile Health technologies extend the reach of care beyond traditional settings. Prescription Drug Monitoring Programs (PDMPs) promote safe and effective controlled substance management, while Results Notification systems ensure the timely and accurate communication of lab, radiology and pathology results.

Quality Reporting/Quality Measures

Quality measures are tools that help us measure or quantify health care processes, outcomes, patient perceptions and organizational structure and/or systems that are associated with the ability to provide high-quality health care and/or that relate to one or more quality goals for health care. These goals include: effective, safe, efficient, patient-centered, equitable and timely care.

Clinical decision support (CDS)

Clinical Decision Support (CDS) systems provide relevant knowledge and patient-specific information to clinicians, staff and patients at the right time to improve health care decisions. CDS includes various tools that enhance decision-making within the clinical workflow.

Clinical Registries

Clinical Data Registries and Qualified Clinical Data Registries may be used to capture data from your EHR and report clinical quality measures. Some states require clinicians to report specific diseases and conditions, and/or to record patients’ immunizations into a registry.

Remote Patient Monitoring /Telehealth / Mobile Health

Remote Patient Monitoring, Telehealth and Mobile Health technologies enable health care providers to monitor patients' health in real-time, facilitating timely interventions and extending care beyond traditional settings. These tools enhance access to care, particularly for patients in remote or underserved areas, by offering continuous monitoring and virtual consultations.

Prescription Drug Monitoring Program

A Prescription Drug Monitoring Program (PDMP) is a state-run electronic database that tracks the prescribing and dispensing of controlled substances. It helps health care providers identify potential prescription drug misuse and supports safe prescribing practices.

Clinical Alerts

Allows clinicians to be notified of patient test results as well as admissions, discharges, or transfers between facilities - to improve care coordination and timeliness of care.
