People who receive care and their families are partners in quality improvement.
It is important to Superior Health to include Medicare beneficiaries and their families in discussions about how to improve the care they receive. Together we will work to:
- Improve the quality of care delivered in nursing homes.
- Decrease opioid misuse and improve behavioral health.
- Increase chronic disease prevention and self-care management.
- Increase patient safety.
- Improve care transitions – moving patients to and from different settings of care.
Person and Family Engagement (PFE) is a critical strategy that ensures involvement of patients, residence and their families in the design, delivery and evaluation of their care. The goal of PFE is to put people first and engage them in all aspects of quality improvement and care. PFE can not only improve health, but also help advance health equity.
Superior Health helps promote proactive communication, partnered decision making and collaboration across health care players, patients, residents, family advocates and community allies on strategies that promote person-centered care. To ensure these diverse voices are incorporated into efforts to improve care in organizations across the care continuum, Superior Health established the Person and Family Advisory Council (PFAC).
This video presentation helps viewers build a family council foundation through learning to describe resident rights to meet as a group and the related organizational responsibilities; recognizing the benefits for residents and families in being more engaged with you as a partner; and list the benefits for your organization in engaging residents and families in your quality and safety work.
