Health disparities are preventable and measurable differences in the burden of disease, injury, violence or opportunities to achieve optimal health that are experienced by populations that have been disadvantaged by their social or economic status, geographic location and environment.

Superior Health designed the Community Health Disparities Reduction Partnership (CHDRP) Initiative to invest in projects in Wayne County (Michigan), Ramsey County (Minnesota) and Milwaukee County (Wisconsin) aimed at reducing disparities in health care outcomes in these three communities.
