Community Coalitions for Improving Care

The Superior Health Quality Alliance (Superior Health) Community Coalitions for Improving Care initiative connects health care organizations. Together, with the common goal of improving health and health care available for all in your community, you will collaborate, share best practices and network with peers in your community.

Coalitions in Superior Health, are provided:

  • Opportunities to work with real-time data
  • Assistance with the evaluation of data
  • Opportunities to be a part of large-scale systems of change
  • Opportunities to use data to help your group focus on shared areas of improvement and provide relevant national best practices, educational
    resources and tools
  • Access to subject matter experts on quality improvement processes and regulatory reporting requirements
  • Learning opportunities offered in a variety of ways that fit skill sets and are easily accessible
  • Alignment to build on existing quality and safety improvement work across the care continuum

Promotional Media Toolkit – Easily Promote Your Facility!

This package was developed to help you promote your decision to participate with Superior Health Quality Alliance. You may use the tools to announce your participation to the public, your staff, clients/patients/residents, stakeholders and more.

Sharing your participation internally can help build excitement, participation and buy-in. Sharing your participation externally lets the public know your organization cares about them and is willing to dedicate resources to be at the forefront of improving care and their health care experience.

All the materials have been created to allow for fast and easy localization for self-promotion. You may choose to use any of the materials that fit your needs. Simply localize the materials with the appropriate information about your organization in the marked spaces.

The Focus of Our Work

Superior Health is powered by eight organizations with proven success driving achievement of Medicare quality improvement program goals. Superior Health will work to improve the quality of health and health care for health care consumers, patients, clinicians, health care organizations and communities by supporting these five areas:

  • Improve nursing home quality focusing on reducing unnecessary resident harm, reducing hospitalizations and increasing a facility’s Five Star Quality Rating.
  • Increase quality of care transitions through engagement of community coalitions to reduce unnecessary emergency department visits and increase medication safety.
  • Increase chronic disease prevention and self-care for those living with cardiovascular disease, diabetes and chronic kidney disease.
  • Increase patient safety including reducing adverse drug events and healthcare-related infections.
