Advance care planning can include different types of documents. Provider Orders for Life Sustaining Treatment (POLST) is a standard for translating patient preferences (often recorded in a health care directive) into medical orders, which will be followed by EMTs and across health care sites. POLST is often used inappropriately or not used at all due to a lack of understanding of its purpose. In 2016, the Minnesota POLST form was revised with the goal of correcting some of the issues Minnesotans currently face.

Audience

Palliative care, home health, and long term care providers.

Objectives

  • Understand the differences between a directive and a POLST form, and why they are significant and are not mutually exclusive documents
  • Describe the five sections of the revised POLST and explain the intent of each
  • State common issues with POLST implementation and recommended solutions to address the issues
