This webinar will introduce all participants to the National Nursing Home Quality Care Collaborative (NNHQCC). The goal of the NNHQCC is to improve systems of care that lead to better quality outcomes for your nursing home residents.

Objectives:
Participants will be able to:

  • Identify goals of the National Nursing Home Quality Care Collaborative
  • Describe NNHQCC participation expectations
  • Identify upcoming NNHQCC learning opportunities

Speakers:
Liz Dominguez, BSN, RN, serves as a lead quality consultant for MetaStar nursing home quality improvement initiatives. In this role, Liz provides consultation and coaching to nursing home teams regarding quality improvement for clinical and non-clinical topics. Liz has a background in geriatrics and general medicine, and has been employed in the Healthcare industry for over 11 years as a certified nursing assistant and registered nurse in both the nursing home and hospital setting.

Kristi Wergin, BSN, RN, CPHQ, is a program manager at Stratis Health. In this role, she provides quality improvement leadership for Minnesota nursing homes as part of the Centers for Medicare and Medicaid Services (CMS) Quality Innovation Network Quality Improvement Organization contract. Kristi has held both direct care and leadership roles in a geriatric setting for 25 years. She currently leads the MN Partnership to Improve Dementia Care in Nursing Homes.

Donna Beebe, BSN, RN, C, is MPRO’s lead quality improvement coordinator for nursing home quality initiatives.  Since 2002, she has assisted CMS Quality Initiative nursing home participants with quality improvement efforts for clinical and organizational topics. Donna also has many years’ experience assisting providers from across the continuum of care, stakeholders and community organizations to successfully collaborate, work to improve cross-setting transitions of care and reduce hospital readmissions. Donna has been active in Michigan’s long-term care arena for over 30 years; she has held both leadership and front-line positions.
