Join Lake Superior QIN on July 15 for the official kick-off of the Cardiac Healthcare Learning and Action Network. Our first webinar in this multiyear series will provide an overview the important facets our partnership to improve cardiac health in rural and underserved communities in Michigan, Minnesota, and Wisconsin. Whether you are part of a physician office, large medical group or home health agency, disparities in the care provided to different patient populations across our three states ultimately affect all of us.

Webinar Objectives

After attending this webinar, participants will be able to:

  • Understand the range and impact of cardiovascular care disparities in underserved communities
  • Describe the key goals and foundation of the national Million Hearts® initiative
  • Describe the overall objectives of the Improving Cardiac Health and Reducing Cardiac Healthcare Disparities initiative
  • Describe the role of home health agencies in the program
  • Understand the patient-centered and team-based approach to improving cardiac health in underserved settings

Target Audience

The target audience for this webinar includes: Clinical Outcomes Coordinators, Community Health Workers, Director of Quality, Medical Directors, Nurse Mangers, Nurse Practitioners, Patient Advocates, Practice Administrators, Physicians, Physician Assistants, and Physician Office Staff
