Are you maximizing all the resources available to your patients who have diabetes? There are a variety of low cost and underutilized resources available to patients. Learn more about evidence-based diabetes education to help improve patient outcomes. This presentation will provide information about how to find and connect your patients. For your convenience, four separate event times area available on this day.

Audience

Note that each webinar will be tailored for different audiences but anyone is welcome at any of the times listed. Please review the options below to choose the best time for you.

  • 10:00 – 11:00 a.m. CT: Quality managers and payer systems.
  • 11:30 a.m. – 12:30 p.m. CT: Providers, nurses and care coordination staff who work with patients with diabetes. Providers, nurses and Quality Managers in hospitals.
  • 1:00 – 2:00 p.m. CT: Palliative care, home health and long term care providers. Social workers and care coordinators may also be interested.
  • 2:30 – 3:30 p.m. CT: Providers, nurses and Quality Managers in hospitals and in-patient psych facilities.

Objectives

  • Understand the value of augmenting clinical diabetes support with community-based self-management education.
  • Recall available evidence based self-management interventions for patients.
  • Access and utilize a new tool for referral to the evidence based self-management.

  2. Click “Register” on the left.
  3. Enter the information as prompted.
  4. You will receive an email with the call and login information.
  5. Note: This email may go to your spam or junk folder. It will come from the email address messenger@webex.com.

Join

  1. Visit the registration and join page for your preferred time listed above..
  2. Click “Join” (located to the right of the event title).
  3. Enter your name, email address, and registration ID that was sent to you after registration as prompted.
  4. Enter the password: Diabetes
  5. The automatic system set-up should start at this point. If a dialogue box appears, click run. Please note, the automatic system set-up does take a few minutes to complete.
