LEARNING OBJECTIVES:
- Describe a specific strategy to identify what matters most to each and every patient,regardless of care providers, settings and time.
- Discuss the importance of health literate care and tools which support person-centered care.
- Review how “Always Events” can improve communication and promote person-centered care.
OUTCOME STATEMENT:
The purpose of this session is to prepare healthcare quality improvement professionals to implement person-centered care strategies by exploring promising practices for the identification of what matters most to each and every patient, regardless of care provider, setting, and time. We expect that this experience will help participants demonstrate and promote person-centered care successes and identify opportunities for improvement, all of which may promote advances in care that impact the Medicare beneficiaries served by the work of the QIO Program.
