The American Heart Association and American Medical Association partnered to launch Target: BP in 2015 to improve blood pressure control and build a healthier nation. This national initiative aims to reduce the number of Americans who have heart attacks and strokes by urging medical practices, health service organizations, and patients to prioritize blood pressure control. Based on the most current AHA guidelines, Target: BP supports physicians and care teams by offering access to the latest research, tools, and resources to reach and sustain blood pressure goal rates of less than 140/90 mmHg within the patient populations they serve.

Audience

Physicians and care teams.

Objectives­

Following the webinar participants will understand:

  • What the Target: BP Program entails.
  • Why a clinic should participate in the Target: BP Program.
  • Where to find and how to use the Target: BP Program tools and resources.

Speakers

Sueling Schardin, MPH, RD, is the senior community health director with the American Heart Association. She has worked in the cardiovascular health area for over twelve years with the American Heart Association and the Minnesota Department of Health.  She received her masters of public health and completed her dietetic internship at the University of California, Berkeley.

Stephanie Worth. Stefanie Worth has spent the last six years of a 25-year career in the nonprofit sector at the American Heart Association. She leads the Detroit market’s Community Plan priorities as senior community health director. Stefanie is responsible for translating health aspirations and goals into actionable tactics for all levels of volunteers, fostering understanding of these tactics among staff, and working within the community to help AHA reach its population health and prevention goals.
