RegisterImproving the quality, safety, and value of the care patients receive through the Medicare program is what the QIO Program is all about. This six-webinar series highlights the innovative work the Lake Superior QIN Coordination of Care (COC) Initiative partners across care settings and in the community have done to improve transitions of care and reduce readmissions.

Part 1: Using Teach-back Across the Continuum
Teach-back is a strategy used to increase patient and family engagement in their health care. Hear how the Lake Superior QIN Fargo/NW Minnesota COC Community chose to utilize this intervention across the continuum of care – including hospitals, home health agencies, hospice programs, public health, skilled nursing facilities, and senior services groups. Challenges, successes, and outcomes will be shared during this presentation.

Part 2: Stakeholder Engagement Strategy for Post-Acute Care Older Adult Behavioral Health Video Training Series
How does an off-the-cuff idea from a presentation question and answer period result in an innovative three-episode video training series? Learn how members of the Lake Superior QIN East Metro Coordination of Care Steering Committee recruited key stakeholders and content experts to produce a video training series focused on improving behavioral health outcomes by boosting caregivers understanding of older adult behavioral health.

Audience

Health care and community-based organization professionals from across the continuum of care (hospitals, home health, skilled nursing, clinics, etc.) who are interested in improving care transitions with the goal of reducing readmissions.

Objectives

  • Identify how the use of the teach-back technique prevents hospital readmissions and improves patient satisfaction scores
  • Describe a post-acute setting video training series to prevent and de-escalate behavioral crises before they become a major event
