Self-management education is an evidence-based, proven method for improving chronic conditions. Please join us to discuss Michigan’s Personal Action Toward Health (PATH) statewide program, based on Stanford University’s Diabetes Self-management Program (DSMP).
PATH is a six-week workshop for adults with chronic conditions including type 2 diabetes, their family members and caregivers. Two trained lay leaders guide each workshop, meeting once a week for 2 ½ hours in community settings, such as senior centers, libraries or churches. Workshops are also be held in health care provider offices. Learn more.
Audience
Michigan health care providers including physicians, nurses, medical assistants, physician assistants and other clinical or office staff. Public health professionals, community health workers (CHW) and others who work with patients suffering from diabetes and other chronic conditions.
Objectives
- Define evidence-based programs and their benefits to patients
- Discuss the PATH program in Michigan
- Provide testimonials from past participants
- Show how PATH can benefit your patients in the management of their chronic condition(s)
- Identify how you or your organization can get more involved in the PATH program
