Join us for a webinar overview of the new measures, plus a look at how ProHealth Care System Milwaukee handles transitions of care, including bridge appointments, creating a mobile psych department, and setting up triage protocols in the ED.
Participants will be able to examine new transitions of care measures for the Inpatient Psychiatric Facility Quality Reporting (IPFQR) that will be introduced by CMS in July as well as learn a best practice related to outpatient visits with a mental health provider within 30 days after an IPF discharge.
