Are you aware of the new quality reporting measures coming in July 2016 for inpatient psychiatric facilities? Join us for a webinar overview of the new measures, plus a look at how ProHealth Care System Milwaukee handles transitions of care, including bridge appointments, creating a mobile psych department, and setting up triage protocols in the ED.

Participants will be able to examine new transitions of care measures for the Inpatient Psychiatric Facility Quality Reporting (IPFQR) that will be introduced by CMS in July as well as learn a best practice related to outpatient visits with a mental health provider within 30 days after an IPF discharge.

Audience

Nurses, Clinical Data/Performance Analysts, Director of Quality; Director of Nursing/Patient Care; Director of Patient Services; Hospital Quality Directors; Transition of Care Coordinators, and any health professionals with an interest in improving care related to the topic area are welcome to attend.

Objectives­

Following this webinar, participants will be better able to:

  • Discuss the current transitions of care measures for IPFQR and the new measures which will be replacing them
  • Create awareness of the new IPFQR transitions of care measures which will be collected beginning in July 2016.
  • Describe one best practice for outpatient follow-up visits post-IPF discharge with a mental health provider

1.0 contact hour will be awarded for successful completion.

Registered Nurses – Minnesota:

This program for 1.2 contact hours has been designed to meet the Minnesota Board of Nursing continuing education requirements. However, the nurse is responsible for determining whether these activities meet the requirements for acceptable continuing education.

MetaStar is the approved provider of nursing contact hours in a joint planning agreement with the Lake Superior Quality Network (LSQIN).
