Presented by the Mental Health Community Partners Network and Institute for Clinical Systems Improvement (ICSI), this event is the first in a series of opportunities focused on care transitions for people who have mental illness. Health care and community organizations, consumers, and family are invited to rally around and plan for optimal care transitions for patients who have mental illness. Representatives of hospitals, health systems, community organizations, mental health clinics, primary care clinics, health plans, consumers, and family members are welcome to participate.

Use this time as a platform for local change while learning from others from across the state. If you have a role in the transition of care for patients with mental illness, this event is for you.
