Welcome to the National Nursing Home Quality Care Collaborative’s Learning Session Six. In webinar 3, nursing homes will share strategies they are using to reduce hospitalizations. Strategies you will hear about include data mining, assessment and early intervention, staff and family education, and advance care planning.

Objectives­

  • Describe strategies nursing homes are using to prevent hospital admissions
  • Describe measures nursing homes are using to identify if strategies resulted in improvement

Speakers

Michelle Sholten, RN, LNC, is quality assurance director/assistant director of nursing for Resthaven. Shellie has worked in LTC for almost 30 years. She has been in her current role for 20 years. Before that, she was a floor nurse, a nurse educator, and a DON. Shellie also is a legal nurse consultant and serves as an expert witness in LTC medical malpractice cases.

Season Marinich, MSN, RN, is the director of nursing at Bronson Commons Post-Acute and Skilled Nursing Facility in Mattawan, MI. She has been a nurse for 17 years with a background in acute care for 14 years in a variety of settings, including med-surg, emergency, and trauma and three years in post-acute care.

Nancy Stratman, Cuyuna Riverwood. Since January 2014 Nancy Stratman has been a member of the Senior Executive Team at Cuyuna Regional Medical Center in Crosby, MN, where she serves as senior services administrator.  She oversees the critical access hospital’s attached Care Center consisting of 113 bed skilled facility (including TCU and memory care) and the organization’s culinary services.  She has been a long-term care administrator for 18 years serving in rural Minnesota communities. Nancy is the chair-elect for Leading Age MN board, the largest association of organizations serving Minnesota seniors.   She is currently the long-term care representative and former chair of MDH Rural Health Advisory Committee, having held that seat for eight years.
