Janelle Shearer, RN, MA, CHPQ, will lead a breakout session entitled “Community Partners Working Together to Optimize Hospice Use.” The session will take place as part of the Minnesota Network of Hospice & Palliative Care 2015 Conference, “Joining Knowledge, Passions, and People,” April 19-20, 2015. The goal of the 2015 MNHPC Conference is to advance the knowledge of health care professionals who provide end-of-life and serious illness services, resulting in improved quality of care for patients, families, and communities.
