Heart failure is one of the top diagnoses involved with readmissions throughout the Great Plains and Lake Superior Quality Innovation Networks. This webinar is the sixth in a series to assist communities in this seven-state region improve heart failure care.

When heart failure progresses to an advanced stage, difficult decisions must be made. Is quality of life more important or is lengthening life through aggressive treatment the goal? These decisions aren’t easy. A partnership between patient and healthcare provider in which the medical options are honestly discussed and decisions are made based on what the patient wants is the goal. This webinar will describe the options for managing CHF when symptoms are difficult to control. Steps on how to develop a treatment plan with the patient to achieve those bucket list moments will also be discussed.

Audience

Health care professionals interested in improving heart failure care including, but not limited to: nurses and medical providers in hospitals, clinics, nursing homes, home health agencies and other community settings.

  • Discuss CHF and management of chronic symptoms
  • Identify the steps in talking to patient and family about end of life care

Speaker

Penni Weston, MS, FNP-BC, 7 Day Clinic, Essentia Health Fargo, ND
Penni Weston, MS, FNP-BC, has served as a Health Services Nurse Practitioner for Essentia Health in Fargo, ND, since 2011. She provides walk-in and urgent care services to patients ages birth to adult at the 7 Day Clinic. Penni received her Family Nurse Practitioner Certificate and Master of Science degree from the University of North Dakota in Grand Forks, ND. She is certified as a Family Nurse Practitioner and in Gerontology both through the American Nurses Credentialing Center. Penni has served in various capacities in medical facilities including long term care administrator in both North Dakota and Ohio.
