Heart failure is one of the top diagnoses involved with readmissions throughout the Great Plains and Lake Superior Quality Innovation Networks. This webinar is the second in a series of webinars to assist communities in this seven-state region improve heart failure care.
The clinical status of a heart failure patient is negatively impacted by non-adherence to the self-care demands of this disease. To address this problem we created a heart failure toolkit to assist patients with heart failure maintenance, management and confidence. This presentation will review how one idea for how to ensure safe transitions and reduce readmissions can grow into a program using data to drive the initiative forward. We will also discuss the use of common cause analysis to understand opportunities for improvement in the areas of readmission.

Health care professionals interested in improving heart failure care including, but not limited to: nurses, HF program coordinators, cardiologists, program medical directors, quality improvement personnel.

Speaker
Karen Bush, MSN, FNP, BC, NCRP, President, Selfcare Solutions LLC
Karen Bush has over 25 years of experience in health care. She began her career as an ICU and ER nurse in both urban and rural settings. In 2003 she earned a Masters in Nursing from Saginaw Valley State University and became a Board Certified Family Practice Nurse Practitioner. She practiced as a primary care provider in the outpatient and inpatient arena with experience in hospice, hospital medicine and long term care. She recently worked for Covenant HealthCare where her primary responsibility was to drive the development and implementation of the transitions of care initiative within the hospital in addition to creating and functioning in the role of a Transition Coach. She has achieved a National Certification in Readmission Prevention. Currently, she is employed with Health Trust as a Director for Clinical Data Solutions where she assists hospitals to utilize data to identify opportunities to improve patient care. As the president of Selfcare Solutions LLC, Karen continues to work to assist health care providers reach out to their patients with meaningful information and tools for self-management.
