The national QIN-QIO program’s Strategic Innovation Engine (SIE) is launching a Call for Innovative Practices.

The SIE’s mission is to rapidly move innovative, evidenced-based quality practices from research to implementation through the QIO-QIN program and the greater health care community. It seeks to advance CMS’s three part aim – better health care, better health and smarter spending.

The SIE process is to 1) Identify, 2) Evaluate, then 3) Spread innovations. Identify begins with a Call for Innovative Practices from a variety of health care settings and organizations. The online application process will open in early June; the submission deadline is Friday, July 1, 2016. In the meantime, download the application to begin collecting the necessary information.

Those working to improve the health care system: providers, payers, patient/families, associations, and innovation labs.

Focus five high impact topic areas:

  • Managing multiple chronic conditions
  • Engaging patients and families as active members of the care team
  • Coordinating care within and across settings
  • Streamlining patient flow
  • Integrating behavioral health into care
