Chronic obstructive pulmonary disease (COPD) is one of the top diagnoses involved with readmissions throughout Minnesota. Lake Superior QIN, Minnesota Hospital Association, and the American Lung Association Minnesota Chapter are partnering to provide a webinar series to inform providers across the continuum of care in support of patients with COPD. Pulmonary Rehabilitation is one key element to successful treatment and care for patients with COPD. There are many options available throughout the state of Minnesota, and we will hear from one featured program at CentraCare St. Cloud. During the session, you will learn what pulmonary rehabilitation is, who benefits from it, and how it supports the family and friends of the person with COPD as well as the outcomes and benefits from the completion of a pulmonary rehabilitation program.
