Chronic obstructive pulmonary disease (COPD) is one of the top diagnoses involved with readmissions throughout Minnesota. Lake Superior QIN, Minnesota Hospital Association, and the American Lung Association Minnesota Chapter are partnering to provide a webinar series to inform providers across the continuum of care in support of patients with COPD. Advanced COPD is an inherently palliative disease, with treatments focused on relieving symptoms and improving quality of life. This webinar will review data to address important issues, including predicting prognosis, engaging patients in advance care planning, and relieving dyspnea in advanced disease.
Objectives
- Learn about improving prognosis prediction in COPD patients
- Discover how to engage patients in advance care planning
- Understand what works and what doesn’t in relieving dyspnea in advanced COPD
