This webinar will describe a unique medication therapy management program in which the Wisconsin Pharmacy Quality Collaborative (WPQC) network of pharmacists has provided comprehensive medication reviews to WEA Trust members with hypertension and risk of low medication adherence.

  • Describe the history of the Wisconsin Pharmacy Quality Collaborative (WPQC) program
  • Discuss the targeted collaboration between WEA Trust and WPQC
  • List outcomes of the WEA Trust/WPQC hypertension pilots
